Open or public records specifically associated with the Oklahoma Employment Security Commission may be requested pursuant to the Oklahoma Open Records Act. This act is a series of laws designed to guarantee that the public has access to public records of governmental bodies in Oklahoma. To make a request, submit the form below.
This request is only for records associated with the Oklahoma Employment Security Commission and does not include records for other agencies or the state of Oklahoma as its own entity. To obtain records specific to an unemployment claimant or related employer, please follow the instructions in the section below.
All requests for individual records must contain the Social Security Number of the individual that records are being requested on. Request for employer records must contain the employer's UI Tax Account Number. Both of these requirements are included in the Waiver of Confidentiality forms. All Waivers must be signed by a notary and the original submitted to OESC's Custodian of Records. All court order requests should include a cover letter with the required information (SSN/Account Number), because court orders will not publish this information.
There is a $25.00 per hour (with a minimum of one hour charge) search fee, plus $0.25 for each page copied on each record request. If certified copy of records are requested, there is a $1.00 certification fee. An invoice will be sent once the documents are retrieved.
